Abstract

The present study examined the antimicrobial resistance characteristics of Salmonella enterica serovar typhi isolated from blood samples of patients who have been clinically diagnosed with typhoid fever in Misan province, Iraq. From November 2024 and May 2025, a total of 187 blood samples underwent analysis through automated blood culture systems, resulting in 105 confirmed cases of S. typhi. The isolates were subjected to biochemical identification and antimicrobial susceptibility testing in accordance with CLSI 2025 guidelines, utilizing the Kirby-Bauer disk diffusion method. The results indicated a significant level of resistance to ampicillin (95.2%), ciprofloxacin (98.1%), and ceftazidime (98.1%). In contrast, complete sensitivity was observed for ertapenem (100%), with nearly complete sensitivity for imipenem and piperacillin-tazobactam (99.05%). The demographic analysis revealed a higher prevalence among males between the ages of 21 and 30 years. The analysis of seasonal pattern indicated a surge in cases throughout the winter months. The results demonstrated a significant public health issue related to multidrug-resistant S. typhi strains, emphasizing the importance of ongoing antimicrobial monitoring, careful antibiotic application, and improved diagnostic systems to facilitate targeted treatment strategies in endemic areas such as Misan province, Iraq.
